CD103+ dendritic cells (DCs) carry bacteria from the small intestine and can present antigens to T cells. Yet they have not been recorded sampling luminal bacteria or presenting bacterial antigens in mesentery lymph nodes. We used 2-photon microscopy in live Cx3cr1(+/gfp) Cd11c-YFP mice to study these processes. At steady state, sparse CD103+ DCs occupied the epithelium. They patrolled among enterocytes while extending dendrites toward the lumen, likely using tight-junction proteins to penetrate the epithelium. Challenge with Salmonella triggered chemokine- and toll-like receptor (TLR)-dependent recruitment of additional DCs from the lamina propria (LP). The DCs efficiently phagocytosed the bacteria using intraepithelial dendrites. Noninvasive bacteria were similarly sampled. In contrast, CD103+ DCs sampled soluble luminal antigen inefficiently. In mice harboring CD103+ DCs, antigen-specific CD8 T cells were subsequently activated in MLNs. Intestinal CD103+ DCs are therefore equipped with unique mechanisms to independently complete the processes of uptake, transportation, and presentation of bacterial antigens.

Objective:  Increasing evidence supports the effectiveness of comprehensive early intervention at first onset of psychotic symptoms. Implementation of early intervention programs will require population-based data on overall incidence of psychotic symptoms and on care settings of first presentation.

Methods:  In five large health care systems, electronic health records data were used to identify all first occurrences of psychosis diagnoses among persons ages 15-59 between January 1, 2007, and December 31, 2013 (N=37,843). For a random sample of these putative cases (N=1,337), review of full-text medical records confirmed clinician documentation of psychotic symptoms and excluded those with documented prior diagnosis of or treatment for psychosis. Initial incidence rates (based on putative cases) and confirmation rates (from record reviews) were used to estimate true incidence according to age and setting of initial presentation.

Conclusions:  When all care settings were included, incidence of first-onset psychotic symptoms was higher than previous estimates based on surveys or inpatient data. Early intervention programs must accommodate frequent presentation after age 30 and presentation in outpatient settings, including primary care.

While ovarian cancer is recognised as having identifiable early symptoms, understanding of the key determinants of symptom awareness and early presentation is limited. A population-based survey of ovarian cancer awareness and anticipated delayed presentation with symptoms was conducted as part of the International Cancer Benchmarking Partnership (ICBP).

Many symptoms of ovarian cancer are not well-recognised by women in the general population. Evidence-based interventions are needed not only to improve public awareness but also to overcome the barriers to recognising and acting on ovarian symptoms, if delays in presentation are to be minimised.

Random probability sampling was used to achieve a population-representative sample using electronic telephone directories as the sampling frame. The final two digits of each selected telephone number were replaced with two random numbers, to include numbers that were not publicly available. Households were eligible if one or more person was aged 50 or over and spoke English. Where more than one person was eligible, the Rizzo method was used to randomly select one person to be interviewed, thereby giving an equal chance of selection to all eligible people living in the household [34]. Survey data were collected during May to July 2011 using computer-assisted telephone interviews. At the end of the interview, participants were offered contact details of a local cancer support charity.

As shown in Table 2, most respondents were aged over 60 years and of white ethnicity. Half the sample was not married or cohabiting, more than half had been educated up to 16 years only, and almost a quarter had experience of ovarian cancer. Most women anticipated presenting within one week of noticing a potential ovarian symptom.

Health beliefs relating to perceived benefits of early presentation were not statistically associated with symptom awareness or anticipated delay. Overall, women perceived their risk of ovarian cancer to be average/low and held positive beliefs, reflecting the overall lack of concern about ovarian cancer within a population sample. While a moderate amount of concern or worry may have a beneficial role in prompting health behaviour [47], it was not possible to test this due to floor effects (i.e. very low worry scores). Comparison with women at increased risk due to a family history of ovarian cancer would help to illuminate the role of emotions in appraising and acting on ovarian symptoms.

The hypothetical nature of the health threat and cross-sectional design are potential limitations of the current study. Since intentions do not always translate into actual help-seeking behaviour [48], the relationship between cancer symptom awareness and actual presentation would ideally be tested in large-scale prospective studies [49, 50]. The limited association that was found between ovarian symptom awareness and anticipated delay contrasts with Robb et al. [3], who found a modest significant association between higher recognition of general cancer symptoms and shorter anticipated presentation. This may reflect the use of an aggregated ovarian symptom recognition measure in the current study, which may have diluted any effects of specific symptom recognition [51].
