If your patient lives in Fort Collins:
1.Call Major Medical at 970-484-6616 (1420 Riverside Ave #108) and confirm they have a bili blanket in stock. If so…
2. Use "Route" / Free Text / Fax number to use Right Fax to send your Epic order AND bilirubin lab results to: 970-224-2883
3. Notify family of where to pick up the blanket (address listed above). Major medical will typically call the family when the blanket is ready to be picked up, the patient’s phone number auto populates on the printed Epic order. Please confirm with Major medical that they plan on calling the patient AND that they have the patient’s phone number.
If your patient lives in Greeley:
Same instructions but the fax number for that location is 970-584-1021, and the phone number is 970-584-1020
Do this:
biliblanketbaby: delivers to their house within 2-3 hrs and is generally covered by medicaid! social work might usually do this but this website would be helpful if you are trying to coordinate out of hospital.

It has an order form that is super easy to fax or email (I did it on my computer in like 3 minutes)
