The Form DH 680, Florida Certification of Immunization, must be used to document receipt of immunizations required for entry and attendance in Florida schools, childcare facilities, and family daycare homes.

Vaccine Storage and Handling

Requirements necessary to ensure proper vaccine storage and handling include appropriate refrigeration at specific temperatures. When recommended temperatures are not maintained, vaccines may lose their potency and result in the recipients being inadequately immunized. Our staff performs on-site immunization reviews at VFC provider clinics to ensure that vaccines are stored properly. Fully online students who will not be taking any courses on a UWF campus or location are not required to provide proof of immunization but should submit a waiver through the MyUWF Immunization Status App. If the student enrolls in a face-to-face course or has to come on campus for any reason, the student will be required to submit the required immunization documentation.

Students wishing to decline this vaccine must read the CDC's Hepatitis B Vaccine Information Statement, select "Hepatitis B Waiver" and "Decline Vaccine" on the online immunization form indicating they have read the information and still wish to decline the vaccine, and electronically sign the form. Selecting "Decline Vaccine" and signing the form indicate the student understands the possible risk in not receiving the vaccine, including the potentially fatal nature of this disease. If the student is under the age of 18 and wishes to decline the Hepatitis B vaccine, a parent or guardian must provide their signature in addition to the student's signature in order for the waiver to be valid.

Please submit your immunization records through the Student Self-Service Portal at least three weeks prior to orientation and/or course registration. Please allow at least 1-2 weeks for your registration hold to clear after submitting your records.

Unless all immunizations have been recorded in Florida SHOTS, parents/legal guardians are to bring immunization records with them. Note: Not all Florida providers record immunizations in Florida SHOTS. The clinic does not request immunization records on behalf of parents/legal guardians. When moving from out-of-state, it is strongly encouraged that parents/legal guardians obtain a paper copy of all immunizations prior to moving to Florida.

Children under 18 years of age need to be accompanied by a natural or adoptive parent, legal custodian, legal guardian or person who possesses a power of attorney to provide medical consent for the minor. All incoming students must submit a medical health history form, including record of mandatory immunizations, completed and signed by your doctor. All immunizations (including PPD/TB Test) on the Mandatory Immunization and Health Form below are required for this form to be complete. Getting an appointment with your physician can take time, so please schedule your physical as soon as possible.

Below is the process to submit immunization records for transfer. There is a $10 fee due upon submission of forms to complete the transfer. A current working phone number is required to be entered on the immunization records transfer form or transfer will not be processed.

All records will be processed within 48 hours of receipt. A nurse will notify parent/guardian when record is entered into FLShots and advise parent/guardian if child will need additional vaccines. Illegible/incomplete patient information will not be processed.

If you need a copy of immunization records from DOH-Okaloosa, no appointment is required however depending on clinic volume we may not be able to provide the documents immediately and the parent may need to return to pick it up. DH 680 certified immunizations forms are $15

For DH 680 requests, parents may walk-in and complete a request form or download FL 680 Request Form (PDF160KB) and bring to the health department during immunization operating hours. Effective January 3, 2024 there is a $15 charge for records transcription and issuing the certified DH 680 form for each request.

Per USF Policy 33-002, it is mandatory for USF students to submit all required immunization documentation and/or the completed Medical History Form prior to course registration. Course registration will be restricted until you have fulfilled this requirement.

Prior to registration, all new students accepted for admission must submit UCF Mandatory Immunizations form. All students born after December 31st, 1956, must provide proof of immunizations.

UCF has partnered with Med+Proctor for immunization compliance and verification. No forms will be accepted via email due to health information privacy laws. You will not be able to register for classes without completing this requirement. The Immunizations hold on your UCF account will be lifted automatically within an hour AFTER you receive a compliance confirmation email from Med+Proctor. We recommend starting this process early so that you will be able to register for classes during your orientation session.
