Florida State University and the Center for Academic Retention and Enhancement (CARE) are committed to providing access to higher education for educationally and economically disadvantaged students. The CARE Admissions Committee only reviews applications that are complete, containing all required attachments. Please note that acceptance is based on a variety of factors, which include (but are not limited to) academic performance, test scores, and short answer responses. Meeting minimum academic measures does not guarantee admission into CARE. Decisions by the CARE Committee are based on a variety of factors including academic performance, test scores, student short answer responses, educational history, family income, socioeconomic background, and family educational level. All students admitted to CARE must be eligible for the Pell Grant during summer and fall/spring terms.

Both CARE and FERA provide significant discounts to customers who qualify for bill assistance based on their household size and income. A single application takes two minutes to complete. No additional documentation is required.

If you need community nursing home care, domiciliary care, adult day health care, geriatric evaluation, or respite care, you may need to fill out an Application for Extended Care Services (VA Form 10-10EC).

The new Fast Track Child Care Scholarship application process shortens your wait to three (3) business days to get help with child care expenses and 60 days of child care assistance while your Child Care Scholarship application is processed, if you are eligible!

Willing to provide proof that each child needing child care is a citizen of the United States or a qualified alien. (Provide a Birth Certificate for each child or documentation that certifies the child's status.) NOTE: Parents ARE NOT required to be citizens of the United States.

Step 4: If you are eligible, a temporary, 60-day Scholarship will be issued to assist with the cost of child care while we process your full application. We will ask for additional information and documentation to see if you are eligible for a 52-week Scholarship. If you provide the additional information and documentation within 15 days of submitting your fast track application, we will let you know if you are eligible for a 52-week Scholarship before your temporary Scholarship expires.

Families take the scholarship to a child care provider that participates in the Maryland EXCELS program. The scholarship is a signed agreement between the State, provider and parent. The scholarship is returned to CCS Central 2 for processing. The family is responsible for paying the State assigned co-payment and any amount not covered by the Scholarship directly to their child care provider. Once the child begins care, the child care provider submits an invoice for payments and is then reimbursed for the scholarship portion of the payment by the State.

The family co-payment is the amount that the family pays for child care and is an amount assigned by the State. The amount of the co-payment for each child decreases if families are receiving scholarships for more than one child. Families eligible for Temporary Cash Assistance and Supplemental Security Income have a $0 co-payment. Many child care facilities charge an additional amount to families -- this is called an additional fee and is the difference between the amount they charge any family, the reimbursement amount and the State assigned co-pay. Payment information should be clearly addressed in any provider contract.

Provides financial assistance to eligible individuals with Supplemental Security Income (SSI) and other low-income individuals interested in residing in a Department of Social Services licensed assisted living facility or a local department of social services approved adult foster care home

Applying for PeachCare for Kids is part of the overall Medical Assistance application process. When you apply, either online, by phone, by mail, or walking in to a Division of Family and Children Services (DFCS) or Right from the Start Medical Assistance (RSM) Group office, there is one application for Medical Assistance. Please call before visiting to ensure in-person availability. Based on your household composition, income, and other circumstances, we will determine the most beneficial coverage for your child(ren). Children who fall under the Medicaid income limits will be put on the appropriate type of Medicaid. Children who fall within the PeachCare for Kids income limits will be put on PeachCare for Kids. No separate application is needed.

If you're found eligible for coverage, you'll receive a ProviderOne services card in about two weeks. Coverage will begin on the first day of the month you submitted your application. You'll be auto-enrolled into an Apple Health managed care plan.

The trauma care funding program enacted by House Bill (HB) 3588, 78th Texas Legislature, stipulates that 94% of the funds accumulated in the Designated Trauma Facilities and Emergency Medical Services (DTF/EMS) Account be distributed to eligible Texas hospitals to reimburse a portion of their uncompensated trauma care.

HB 2048, which was passed during the 87th Texas Legislative Session, Regular, amended Chapter 780 of the Texas Health and Safety Code use at least 94 percent of the money appropriated from the account, after any amount the commissioner is required to reserve is deducted, to fund a portion of the uncompensated trauma care provided at facilities designated as state trauma facilities by the department or an undesignated facility in active pursuit of designation.

Texas Health and Safety Code 773.122 directs DSHS to use at least 27% of funds in the Emergency Medical services, Trauma Facilities, and Trauma Care Systems Account (Fund 5108); and at least 27% of funds in the Emergency Medical Services and Trauma Care Systems Account (Fund 5007) to fund a portion of uncompensated trauma care provided at hospitals designated by the state as trauma facilities.

NJ FamilyCare - New Jersey's publicly funded health insurance program - includes CHIP, Medicaid and Medicaid expansion populations. That means qualified NJ residents of any age may be eligible for free or low-cost healthcare coverage that covers doctor visits, prescriptions, vision, dental care, mental health and substance use services, and even hospitalization.

DES provides child care assistance to eligible families who participate in employment and specific education and training activities by providing financial support to offset a portion of child care costs for children from birth through age 12.

DES child care assistance can help families pay for care in child care center-based or home settings that are contracted with DES to provide high quality child care. Families are required to cost-share (co-pay) based on family size and income.

DCC also provides child care assistance to individuals receiving Cash Assistance (CA) who are employed or enrolled in the Grant Diversion program, as well as grandparents and guardians when the parent of the child is not living in the same home and when the grandparent or guardian is engaged in an eligible activity.
