New Patient Paperwork

You will be asked to complete paperwork regarding the reason for your appointment and medical history. Please answer the questions as specifically as possible. Understanding your needs and complete medical background allows us to best determine your plan of care. When making your appointment, new patient paperwork will be mailed in advance to expedite your visit.

Insurance Information

A current insurance card is required when booking your appointment and at your visit. The majority of allergy testings and treatment ARE covered by insurance, however as a specialist provider, co-pays for office visits and procedures may differ from your primary care physician. We make every effort to verify insurance benefits prior to your visit but encourage you to check with your company with specific coverage questions. If your plan requires a referral (HMO), this must be obtained through your primary care provider before your appointment. We do not want on administrative issues to limit your time with the doctor. Please note that payment is due at the time of service and the final responsibility rests with the patient.

Allergy testing policy

In most cases testing for environmental or food allergies can be performed at the first visit, however factors that may delay this include medications interfering with test results, insurance restrictions or scheduling conflicts. Tests for medication or insect allergies require special preparation and cannot be performed the same day. If you wish to be tested, avoid antihistamines (Benadryl, Zyrtec, Claritin, Allegra, Azelastine and most other OTC allergy medications) at least 1 week prior to your visit. This is not an exclusive list and some medications may have "hidden" antihistamine effects.

If you are not sure whether a medication should be stopped please contact the clinic. If you are concerned that discontinuing a medication may be uncomfortable or unsafe, it is best to visit with the doctor before stopping them. Our top priority is accurate and safe testing.

Late Arrivals, Cancellations and Missed Appointments

Make every effort to arrive early. Patients arriving more than 15 minutes after the scheduled time may be required to reschedule. Arriving late reduces your time with the doctor and disrupts the care of other patients.

If you must cancel your appointment, please provide us with at least 24 hours advance notice. Appointments are in high demand, and this allows us to see other patients who are waiting for a visit. As a courtesy we try to confirm all appointments the prior business day. Repeated cancellations or no-shows will lead to discontinuation of care from our clinic.
