Please review the policies below to ensure the best possible visit to our practice.
Arrive Early. You will be asked to complete paperwork regarding the reason for your appointment and medical history. Understanding your needs and complete medical background allows us to promptly and appropriately determine the best plan of care. When time allows, new patient paperwork will be mailed in advance to expedite your visit.
Bring current insurance information. Please remember that all insurance companies require that your data be updated prior to each encounter. This usually takes several minutes. If this is not done, your insurance may deny your claim. We do not want time spent on administration requirements to limit your time with the doctor. Please note that payment is due at the time of service.
Late Arrivals. Patients arriving more than 15 minutes after the scheduled appointment may be required to reschedule. Arriving late reduces your time with the doctor and disrupts the care of other patients.
Cancellations / Missed appointments. If you must cancel your appointment, please provide us with at least 24 hours advance notice. Appointments are in high demand, and this allows us to see other patients who are waiting for a visit. As a courtesy we try to confirm all appointments the prior business day. Repeated cancellations or no-shows will lead to dismissal of care from our clinic.
