Master's Thesis Enrollment Form is used to enroll in a Master's Thesis (900) course for the first time. A separate Enrollment or Add/Drop form is not required.

Master's Thesis Activity Report is used by the student to inform the Office of Graduate Academics of the research status. This form is required at the end of each semester in order to get an S/U interim grade for the term.

Master's Thesis Defense Report is to report the final grade (Letter Grade) for the Master's Thesis.
