If your temporary license or certificate for driving is lost, stolen or destroyed, you may obtain a duplicate temporary license or certificate for driving by making application only in person at any driver service center. Holders of a temporary driver license or temporary ID must provide the necessary documents indicating the extended authorized stay in the United States to renew.

During the seven-day period, you may view your electronic driver record as many times as you need to by logging in and choosing the option to View Transaction History. We recommend you save your documents to your computer or device so that you may conveniently view the record at any time. If you are purchasing your driving record for your own reference, you may not need a certified record.

If you are purchasing a copy of your record for a court case, or for an employer, check with the person requesting the record to verify what type of record is needed and if it needs to be certified.

Identity fraud involves someone other than yourself using your identity details. This often happens with a copy of your ID. For example, with a copy of your ID showing your name, date of birth and BSN, fraudsters can apply for a loan or set up a mobile phone contract. As a result, you could receive bills for things you did not buy. Do not let your ID be copied by just anyone. Prevent identity fraud by making a secure copy of your ID.

Is an organisation asking for a copy of your passport, ID card or driving licence? If so, first check whether this is necessary. If you are a third-party and want a copy of an individual's Driver License Record then use the Request for Motor Vehicle Record Containing Personal Information form by using this form. Please see form for any special instructions.

If your driver's license or ID card was lost, a replacement fee is required. However, if the license or ID card was stolen and reported to law enforcement, the fee may be reduced with a copy of the law enforcement report if it is presented when the replacement is requested in a Service Center. CDL holders must provide their SDLA with a copy of their ME Certificate. This information is only being added to the State driving records of CDL holders. Non-CDL holders are not required to self-certify or submit a copy of their ME Certificate to their SDLA. CDL holders, who are found driving in a category other than one to which they self-certified, are subject to suspension or revocation of their commercial driving privileges. CDL drivers, who do not update the expiration date of their ME Certificate with their State, will have their commercial driving privileges downgraded, and will not be eligible to drive a commercial motor vehicle that requires a CDL.

If you are subject to the USDOT medical examiner certification requirement, provide a copy of each new USDOT medical examiner certificate to your SDLA prior to the expiration of your current medical examiner certificate. The medical examiner certificate is often referred to as your "medical card."

An ODR is a certified copy of your driver record that is accompanied by a letter of certification from the BMV. Some employers and school admission offices require a certified driver record. The ODR may be purchased for $4.

A new resident who does not have possession of the out-of-state license or if it has expired, must obtain a certified driving record, dated no more than 30 days from the date of the application, from the state in which they were previously licensed.

If you change your address you are required to notify, in writing, the DMV within 20 days of the change. Failure to do this subjects you to a possible fine or imprisonment. Simply notifying the US Post Office will NOT update your record with the DMV or allow important DMV mail to be forwarded to you. By failing to change your address with the DMV, you could miss the renewal date of your license, or even have your driving privileges suspended or revoked unnecessarily.

The Division may, upon request, renew the driving privileges of a licensee who is temporarily located out of state at the time their license expires. The Division shall not issue a new driver's license to the licensee. The Division will issue a certified driving record to the licensee indicating that the licensee has renewed his or her driving license with a notation of the new expiration date. If you are under the age of 18, a parent, single parent having custody, or legal guardian must sign your application, before you can be tested. Married applicants under the age of 18 are not required to have parental consent, however, you must present a copy of your certified marriage certificate. This permit is not renewable.

The DMV is authorized to impose license restrictions to assure the safe operation of motor vehicles. The DMV may issue you a restricted license with the restriction indicated on the driver's license. Operating a motor vehicle in violation of the restriction is a serious offense and could result in the suspension or revocation of your driving privilege. Once suspended, the DMV does not offer a restricted license for work purposes.

The information you provide when you apply for your driver's license, or apply for a duplicate license must be true and accurate. It is against the law to provide false information when you apply for any type of driving license or identification card, change or alter your license in any way, use someone else's license as your own, or permit someone else to use your driver's license. The penalty for these offenses is a 90-day suspension of your driver's license.

There are many ways you can lose your privilege to drive. Driving while under the influence of alcohol or any other drug, even if it is an over the counter cold remedy or a doctor's prescription, subjects you to the DUI laws. Accumulation of too many citations in a short period of time, driving without insurance, failure to pay any type of court action, failure to supply DMV with current insurance information, and fraudulent checks are just a few of the actions which may result in the loss of your driving privileges. The penalties for driving while suspended or revoked include mandatory jail time and an additional period of suspension or revocation.

A Class D License is a Non-CDL for hire. Every person who is employed by another for the purpose of driving a motor vehicle when in use for the transportation of property for compensation and who does not meet the requirements for Class A, B or C for the commercial driving license needs a Class D license. Exempt from the Class D licenses are vehicles with Class A license plates (regular passenger vehicles).
