Ten years ago, Andrew Farmer (Reed Diamond) married his childhood sweetheart, Beth (Kristin Davis) and the two live in Boston, Massachusetts. Now Andrew is a high-powered literary agent, and his relationship with his wife has not fared as well. After a marital argument about his possible infidelity on Christmas Eve, Beth runs out at midnight and, while trying to retrieve their neighbors' dog in the middle of the street, is killed by a car. An angel, Lionel (Tim Meadows), gives Andrew the chance to relive the last three days as if his wife was alive. However, he cannot change the fate of his wife. There's only one gift he can give to save her life and he only has a very short time to figure out what that gift is. So Andrew spends three days trying to make Beth as happy and figure how to keep her alive, while finding out what he would really be losing.

A recent particularly long stretch of desk sitting, combined with the start of the rainy season in Costa Rica and reading this article about the negative effects of sitting at work for more than four hours a day, left me feeling particularly restless. I was in need of a trip to jump start my spirit, and lucky for me, I found a low cost ticket to Guatemala City with Avianca Airlines that coincided with a three day weekend at work.

Guatemala is home to 22 volcanoes, three of which are active. You can hike Pacaya, which is actually a volcano complex, on a day tour with Guatemalan Adventure. Pacaya last erupted in 2010 and is the most active volcano in the country. You can get remarkably close to the crater on this tour and even roast marshmallows on the hot rocks near the base of the crater. Read about my hike of Pacaya Volcano here.

If you have more than three days to discover Guatemala, you will definitely want to add Tikal National Park to your itinerary. This was one of the largest and most important cities of the Mayan Empire during the Classical Era. (It was also used during film scenes in Star Wars). Tikal is an eight hour bus ride from Guatemala City, or you can fly there and take a day trip.

Interventions:  Patients who had substantially improved after three days' treatment with intravenous amoxicillin were randomly assigned to oral amoxicillin (n = 63) or placebo (n = 56) three times daily for five days.

Main outcome measures:  The primary outcome measure was the clinical success rate at day 10. Secondary outcome measures were the clinical success rate at day 28, symptom resolution, radiological success rates at days 10 and 28, and adverse events.

Results:  Baseline characteristics were comparable, with the exception of symptom severity, which was worse in the three day treatment group. In the three day and eight day treatment groups the clinical success rate at day 10 was 93% for both (difference 0.1%, 95% confidence interval--9% to 10%) and at day 28 was 90% compared with 88% (difference 2.0%,--9% to 15%). Both groups had similar resolution of symptoms. Radiological success rates were 86% compared with 83% at day 10 (difference 3%,--10% to 16%) and 86% compared with 79% at day 28 (difference 6%,--7% to 20%). Six patients (11%) in the placebo group and 13 patients (21%) in the active treatment group reported adverse events (P = 0.1).

Conclusions:  Discontinuing amoxicillin treatment after three days is not inferior to discontinuing it after eight days in adults admitted to hospital with mild to moderate-severe community acquired pneumonia who substantially improved after an initial three days' treatment.
