To ensure that the hospitals do not overcharge and rates do not vary across hospitals, empanelled health care providers (EHCP) are paid based on specified package rates. A package consists of all the costs associated with the treatment, including pre and post hospitalisation expenses. The specified surgical packages are paid as bundled care (explained below) where a single all-inclusive payment is payable to the EHCP by insurer/SHA. The medical packages, however, are payable to the EHCP on a per day rate depending upon the admission unit (general ward, HDU, ICU) with certain pre-decided add-ons payable separately. Day-care packages are payable just like surgical packages. The treatment packages are very comprehensive, covering treatment for nearly 24 specialities that include super speciality care like oncology, neurosurgery and cardio-thoracic and cardiovascular surgery, etc. The package rate (in case of surgical or defined day-care benefits) includes:

Under PMJAY members from families registered under Social, Economic and Caste Census, 2011 (SECC) are eligible for getting benefits in all empaneled hospitals by showing computerized e-cards and photo identity card. Beneficiary of PMJAY from any state having e-card and photo identity proof can avail treatment at any empaneled hospital of any other state.

Step 2-

The Arogyamitra at the network hospitals examines valid ration cards and Photo ID and enrolls the patient along with registration.

The information like admission notes, test done will be captured in the dedicated database by the Medical Coordinator of the Network Hospital as per the requirement of the scheme.

To avail the benefits of Ayushman Bharat Yojana health insurance scheme, it is essential for all the people to check for their names in the Socio-Economic Caste Census-2011 data. This will ensure if their family is eligible to be covered under Ayushman Yojna or not. And only those households whose name is listed in SECC database and active RSBY card holders are entitled to avail PMJAY benefits.

It is important to apply for the Ayushman card as it consists of a dedicated family identification number. AB-NHPM is provided to every beneficiary household. Below are the steps that you can follow to apply or download your Ayushman card online-

You can apply for an e-card once you find out your eligibility for Ayushman Bharat yojana scheme. But before you get this e-card, you will have to go through an identity verification at one of the PMJAY kiosk based on your documents like Aadhaar card or ration card. Even your family identification can be furnished including the RSBY cars, or a govt. certified list of members. After this verification is done, you patient e-card is printed along with your Ayushman Bharat ID, which can be used for future references.

Family identification proofs that can be produced include a government certified list of members, PM letter and an RSBY card. Once the verification is completed, the e-card is printed along with the unique AB-PMJAY ID.

You can access services under PMJAY across the country at any empanelled hospital, irrespective of your home state. All benefits under the scheme will remain the same. Members of a family can avail services in different places/hospitals as e-cards are made individually.

This scheme is entitlement based. No requirement for changing members as there is no limit on number of family members. Family members can be deleted only if proof of death is furnished. Additional family members can be added anytime based on any government approved ID that proves the family relationship (e.g. ration card) for linking a new member to an already verified beneficiary.

PMJAY is an entitlement based mission. There is no formal enrolment process. Families who are identified by the government on the basis of the SECC database both in rural and urban areas are entitled for benefits under this Mission. Rs 5 lakh cover will be provided per year per family. It is recommended to carry Aadhaar as it is a preferred ID to verify if a beneficiary is eligible under PMJAY. However, no services will be denied to any eligible beneficiary if they do not have an Aadhaar card. An eligible beneficiary can also provide other government ID proofs.

This scheme is entitlement based. Once the eligible beneficiary is verified, an individual e-card will be given. The e-card can be made free of cost at the empanelled hospital. A dedicated family identification number will also be allotted to eligible families.

Once you have an e-card. You can use the e-card to avail services across the country, irrespective of which state you belong to. Every time you will not have to go through the process of authentication of whether you are entitled to the scheme or not.

The Arogya Mitra/Operator at Kiosk shall conduct an Individual Identification (based on Aadhaar / non- Aadhaar KYC), Verify Mobile No & Family Identification (Ration Card), Enter name of the family & upload a search on the Beneficiary Identification System (BIS) based on government issued ID proofs furnished by the potential beneficiary. Similarly, beneficiary will also have to submit a proof of family ID card (Ration Card) for establishing his relation in the family. Once the beneficiary is identified it is further sent for approval to the Trust/Insurance company. If the Insurance company recommends for rejection it is the State that will make the final decision about eligibility.

It covers medical procedures including oncology, cardiology, nephrology etc and up to three days of pre-hospitalisation and 15 days post-hospitalisation expenses like diagnostics and medicines are also included in the scheme.
