Similar to Falling Fred, the game begins with Fred falling off a set of four buildings shaped like a square hole. The buildings are a lot brighter and cartoony compared to Falling Fred, which could mean that it is a newer building. However, it changes colors and gets run-down if Fred falls far enough. However, if Fred falls far down the building enough, he will enter "Normal Mode" by falling into a manhole. The manhole is a metallic blue tube instead of a square, but it will change colors like red and dark green the further Fred falls. If Fred continues falling, he will enter "Hard Mode" and he will fall into an unknown cave-like hole. If Fred falls far down enough this hole, he will enter "Nightmare Mode" (the final area) where the cave turns blood-red with white specks, which is supposed to represent Hell.

The game plays identically to Falling Fred, using tilt controls to move Fred. However, there are still obstacles similar to the ones in Falling Fred, but new ones include rotating spikes on the rims (only in normal mode or higher), mid-air bear traps that instantly kill Fred, AC units blowing out lots of air, and more. Also, the shaft Fred is falling down turns into a circle in Normal Mode and higher. However, there are new collectible items that can help Fred, like meteors that make Fred fall very fast safely, hourglasses that slow down time, FRED cubes (heal Fred if all 4 are collected), and even extra Skullies. Conquered nations are forced to provide labor and resources for the war effort, including super-human recruits. Not enlisting after developing powers is treason and will get the person turned into a functional "zombie" trooper. The still free countries have joined together in the Alliance and desperately try to hold Supercity's advance, be it in open combat, with espionage or by researching and developing their own super-human/supertech/mystical assets.

Paradise City: in this variant, Supercity opted for the love and desire of the rest world, becoming the ultimate destination for pleasure and wish fulfillment. Supercitizens use their considerable resources to cater to every vice and fantasy baselines have. This manifests in ways that vary from the mundane (casinos and resorts dot the island) to the exotic (power enhancement, dimensional travel, etc.). Everything has a cost, of course, and is temporary - it's gone after the visitor leaves Supercity (cures for terminal diseases are an exception, for the right price). The mayor is called the Master/Mistress of Ceremonies and supercitizens go out of their way to prevent anything bad from happening to their clients.
