Once you have completed the form, please check to see what approvals are required and forward to that office. For approvals, each office can print, sign and scan form to next approver or can approve in email with form attached. If more than one signature is required, forward the form to each office email until all approvals are made. The last office approving the form will send the form to academic.records@sru.edu.

CPS can provide former students with a copy of a high school transcript, verification of a graduation date from high school, a copy of an elementary school transcript, and an elementary registration card or immunization records.

Records are mailed to your desired location via the US postal service. CPS does not offer a walk-in records request service and does not provide same-day or expedited document return services. CPS does not email or fax records.

If you graduated from a CPS high school within the past five years and are seeking a copy of your high school transcript, or if you have withdrawn from a CPS high school within the past five years, submit a records request directly to your former high school.

For transcript requests, please use the online ordering system (Parchment) link provided on our transcript information page to request a transcript. Transcripts requested to be sent via email will be sent through Parchment during the office closure unless there are issues with the order. Transcripts that need sent by mail and ordered after 8:00 a.m. on Friday, December 22, will not be mailed until Tuesday, January 2, 2023. If you need a transcript immediately, please use the email PDF option on Parchment.

If you need a transcript from Slippery Rock University, please refer to the Parchment online ordering option on transcript request page. This is the best option for students or alumni to request a SRU transcript quickly and easily. Please note: Students with a hold on their account will not be able to complete their transcript request until that hold is resolved.

Registration for courses at Slippery Rock University is completed through the MySRU portal. Important dates for registering, dropping, or withdrawing from courses can be found on the Academic Calendars page. If students need more information about registration, they should consult the Registration resource page, reach out to their academic adviser, or contact the Office of Academic Records and Registration.

The decision to withdraw from one or more courses should always be made in consultation with your academic adviser. In addition to affecting academic progress, full-time students who withdraw from courses and drop below full-time status in a semester may be subject to financial aid consequences. We advise that you contact the Financial Aid office before withdrawing to determine if doing so will affect your aid.

The Course Withdrawal form is fully automated. Once the student submits their request, the form will be forwarded through DocuSign to the course instructor for the student's last date of attendance. The student's academic adviser and the department chairperson of the course will receive a copy of the student's request. Once the instructor approves the request, the form is forwarded to the Office of Academic Records and Registration for processing.

The Office of Academic Records and Registration has various processes for student recordkeeping; view a central list of academic record forms here. If a student needs to forward a form for signature to another academic department, they should use their SRU email account to do so. DocuSign forms have an automated workflow system to route forms to the appropriate individuals for approval.

To obtain a transcript from a California public school, you will need to contact the school directly. If the school is closed, we suggest contacting the local school district or the County Office of Education for assistance. Contact information for these entities may be obtained from the California School Directory.

To obtain a transcript from a California private school, you will need to contact the school or one of its officers directly. Contact information for private schools may be obtained from the California School Directory, or from the Private School Directory data files, which are posted on the CDE Private School web page.

To obtain a transcript from a private or vocational college, you will need to contact the institution directly. If the institution appears to be closed, we recommend that you ensure the institution is closed, as some institutions will close a branch or location, yet remain open. Also, institutions may be a member of a larger organization, therefore we recommend that you try to contact the parent organization, if applicable. If the institution is closed, or not part of a larger organization that maintained their records, you may contact the below organizations to try to obtain your records.

To obtain a transcript from the California Department of Corrections and Rehabilitation (CDCR), Division of Juvenile Justice (DJJ; formerly the California Youth Authority), please contact the CDCR DJJ by phone at 916-322-5759 or visit the CDCR How to Request Transcripts and Certifications from the California Education Authority web page.

You may request an official academic transcript ($10 fee) or an unofficial academic transcript ($5 fee), both of which list all the classes you have taken and the grades you received. Transcripts can be mailed to you or picked up in person from One Stop Student Services.

The University of Alabama at Birmingham will resend a transcript to the same address at no charge after 15 days and/or up to 30 days from the initial date of the transcript order request. Resend requests before 15 days or after 30 days of the initial order request will require additional payment.

Records for students who attended the University of Washington prior to 1983 may be on microfilm. Thanks to the enhancements of Parchment, these students may now request and receive official, electronic transcripts. While placing a transcript order, please provide as much information as possible to assist in locating records, such as:

Students sending their transcripts to application services such as AMCAS, LSAC, NCARB, SOPHAS, AACOMAS or other professional certification, examination, and licensing agencies will be able to provide any application service ID or upload required forms while ordering through Parchment. Parchment provides a list of established agencies to select from within the order form.

The Records Office provides services to faculty, staff, students, and other agencies by informing and safeguarding student academic records in accordance with the Tennessee Board of Regents (TBR) records policies. Receive timely, accurate, and efficient assistance from the initiation of the enrollment process through graduation.

Notify the Records Office once you have cleared your debt and the hold has been released so that we can process the transcript request. We will only hold transcript requests for 30 days from the date of receipt in the Records Office; after that, you will be required to submit another request.

The campus will utilize the National Student Clearinghouse to ensure students have access to transcripts in the event of a catastrophic event or unanticipated closure of the institution. The Transcript Ordering Center can be found at In the event of the campus no longer existing, the System Office will maintain the campus's student records in an archive-only format to service future student records requests.

You may also order a grade report from the Academic Records Office, but please note that grade reports are term specific and do not include all of your academic history. If you need a report of your academic history you will need to order a transcript.



This is important: Not all schools participate in sending transcripts via Electronic Transfer even though they may use one of the services listed above. (For example, some schools using these services will only deliver a paper copy or an electronic PDF version of your transcript.) If your school does not participate in delivery via Electronic Transfer, you must request that your official transcript be mailed to LSAC. If your institution has an established account with LSAC, official transcripts may be uploaded to LSAC via the registrar portal of the LSAC website. Candidates must have official transcripts sent to LSAC for any additional academic work completed, including degrees obtained, to update the file. If your CAS Report has already been sent to requesting law schools, an updated report will be transmitted, provided their terms are still active. Paper copy: Please go to the Transcripts page of your online account. A Transcript Request Form link will be listed below each institution. You may click these links to access your forms. Submit the form to the registrar and request a copy of the transcript be mailed to the address listed on the upper left-hand corner of the form.

You must have your institutions attended send LSAC official institution-issued or attested true copies of your permanent academic record (e.g., transcripts, report cards, examination statements, marks sheets, diplomas) showing subjects or courses studied per each academic term/year with final examination results/grades/marks issued.
