Fully accomplished Personal Data Sheet (PDS) UNDER OATH, using CS Form No. 212 (Revised 2025), accompanied by a passport-sized or unfiltered digital picture taken within the last six (6) months. The required Work Experience Sheet (WES) must also be included.
