Dekalb Approved Proof of Residencies

Cross Keys High School requires 1 proof of residency each year in any of the following combinations:

1. Signed Lease/ signed mortgage contract (must be current)

2. One current utility bills (gas,water, or electric) (disconnect notices not accepted)

3. Notarized Co-Residence Affidavit including:

   1. Owner’s Lease/Mortgage, Utility Bill, and Photo ID

   2. Parent/Guardian proof of residency document (any mailed document to the address on affidavit)
