Crooked, Song, and Tully lakes survey consent form


The Crooked, Song, and Tully lakes watershed surveys are meant to gauge the recreational use and top concerns of the individuals who live, work, or play in the Crooked, Song, and Tully lakes region. Please fill out the corresponding survey of the lakes in which you are a stakeholder. Your participation in the study helps analyze the recreational use of each lake, and general opinions of important stakeholders (people or organizations in the watershed who have a say in local policy making decisions).

Using the responses from the surveys, a management plan will be written to improve the recreational and aesthetic aspects of the lakes.

You must be 18 years or older to answer the survey, and participation in the survey is voluntary. The survey will take about 5 minutes to complete.

You may stop answering survey questions at any time, without consequence.

No personal info will be recorded, and responses are anonymous. Any paper copies of the surveys will be destroyed after the results are digitized. The study represents minimal risk to you.

There is no formal compensation for answering the survey.
