The Chief Minister Relief Fund (CMRF) is a scheme that aims to provide financial assistance to the poorest of the poor who are in distress due to various reasons such as health problems, loss of life or property, natural calamities, accidents, etc. The CMRF is administered by the Government of Telangana under the leadership of Chief Minister Sri K. Chandrashekar Rao.

To apply for the CMRF, one has to fill and submit an application form along with the required documents. The application form can be downloaded from the official website of the CMRF [here] or [here]. Alternatively, one can also obtain the application form from the office of the District Collector or the Revenue Divisional Officer.
The application form consists of two parts: Part A and Part B. Part A contains the personal details of the applicant such as name, address, phone number, Aadhaar number, bank account details, etc. Part B contains the details of the relief sought such as the nature and cause of distress, amount requested, supporting documents, etc.
The applicant has to fill both parts of the application form and attach the relevant documents such as medical reports, death certificates, FIRs, photographs, etc. The applicant has to sign and date the application form and submit it to the concerned authority along with a covering letter. The covering letter should mention the name and address of the applicant, the purpose of applying for the CMRF, and the amount requested.
