Step 2: Complete Required Forms

• Forms are due no later than Thursday, April 10th

Note: if your guest is NOT a Campo student, your guest will need to complete the "Dance Guest Agreement Form", which includes obtaining the signature and business card of an admin at their school. Plan accordingly to meet the deadline as this can take a few days. Please see Parent FAQs for additional detail. Please note this form is not electronic, a hard copy will need to be turned in.
