Nursing Home Profile (searchable directory)
These pages from New York State provide directories of service providers and links to other information about both home health services and institutional services. The Nursing Home Profile directory provides links to individual facilities with quality ratings and links to past state inspections.
Adult Care Facility Profiles (searchable directory)
Similar to the nursing home search link above, but oriented toward assisted living facilities that do not provide skilled nursing care.
This database from the state’s health statistics database Health.Data.NY.gov lists more than 550 care facilities around New York.
Consumer Directed Personal Assistance Program
Information about New York's Medicaid-funded program that provides care services to people who have a medical need for help with activities of daily living (ADLs) or skilled nursing services. Services can include any of the services provided by a personal care aide (home attendant), home health aide, or nurse.
NYS Office for the Aging
Links to a variety of resources and state programs beneficial to older adults
Links to resources – mostly state programs – about long-term care, including a link to NY Connects, a searchable repository of information about long term care services and supports.
Long Term Care Ombudsman Program
State ombudsmen are advocates who help older adults and people with disabilities who live in nursing homes, assisted living, and other licensed adult care homes to understand and exercise their rights to good care in an environment that promotes and protects their dignity and quality of life. The site is searchable by geography for regional (usually countywide) representatives.
Directory of Community Resources on Aging and Disability
This is a listing of community resources (and contact information) to help the public find services for older and disabled New Yorkers. It includes information on: AAAs (Area Agencies on Aging), and organizations that offer access to the state's Health Insurance Information Counseling Program (HIICAP), Long Term Care Ombudsman Program (LTCOP) and NYConnects. The database has three tabs, including ones for the full state, one for the Buffalo area, and one for the Rochester area.
