Application and Assessment

Using Google Forms, we created some examples of surveys that could be utilized throughout the leadership program, from application forms to final assessment. The development team will give full ownership of these forms to the sponsors.

Example Leadership Counselor Application FormThis form is an example of what an application to be a leadership counselor could look like. This form asks more detailed questions than the general participant application in order to get a deeper background on potential leaders in the program.

Example General Application FormThis is an example of the what could be on the application form to be a general participant in the program. This form asks basic questions about contacting the interested party.

Example Mid-Program Leadership Counselor and Mentor Assessment FormThis form was built to allow leadership counselors and mentors to reflect on their role and to identify places where they could improve halfway through the program, as well as give feedback to what could help them more in the role.

Example Mid-Program Assessment FormThis form is built to be shared with general participants for feedback, not mentors or leadership counselors.

Example Final Leadership Counselor and Mentor Assessment FormThis form was built to allow leadership counselors and mentors to reflect on their role and to identify places where they could improve halfway through the program, as well as give feedback to what could help them more in the role.

Example Final Assessment FormThis form is built to be shared with general participants for feedback at the end of the program.
