APPLICATION FORM

Below is an example of an application form. This example shows a model of the form and how it can be properly filled. The form has at least four (04) pages and should be properly cross-checked after filling. Beneath the sample application form are my comments on important aspects to consider when filling.

Important points to Note

Please take not of the following point when filling the application form

  1. "Surname" means your family Name

  2. You MUST make sure that the pictures you use for your application form has EXACTLY the size indicated on the form i.e. 4.5cmX3.5cm. You must insist to your photographer to print your pictures to exactly this size.

  3. Your address should be as detail as possible.

  4. If currently you are not a student or have a job, please indicate in the box "Others" what is it you do.

  5. Make sure all your choices of major selected are related. That is to say, if your first choice is Information Technology, your second and third choice CANNOT be Nursing and Dentistry respectively. These choices are totally unrelated. please take note especially those applying for undergraduate programmes.

  6. The reason for your choice should bring out your motivations and ambition for making the choice. It should also highlights your future goals.

  7. Double check all your punctuation.

  8. If you CANNOT speak and write FRENCH well, don't lie on your form by giving misleading evaluation about yourself. This point is specifically for Cameroonian candidates.

  9. Since most candidates generally don't have TOEFL or IELTS at the time of application, the candidate can just indicate in the "others" box that they have a Native level in English if the candidate has conducted all of their education in English.

  10. Make sure you sign your application form and write the date of signature
