45415 was delivered to VPB-208 of the United States Navy on October 5, 1944. At the time, they were based at Norfolk, Virginia but they were eventually reassigned to Hawaii.
On November 30, a flight of 12 PBM-5 Mariners, split into 4 groups of 3, departed Alameda on a ferry flight to Hawaii where they would be assigned. The weather was rainy with low clouds and poor visibility. 45145 took off alone, and was set to rendezvous with its flight. However, before it could meet up with its flight, it encountered technical issues and was forced to turn around. During its return, it impacted Mt Tamalpais for unknown reasons. All 8 on board died.
The wreckage is only somewhat visible from satellite imagery.
