PA-23-250 MSN 27-7405457 was delivered to a private owner in 1974 with the registration N54164. It was sold to another private owner later that year. At some point (possibly that 1974 sale) it was registered PT-KNZ with a Brazilian owner.

This crash occurred during a flight from St Croix to Port of Spain on February 1, 1976. The aircraft encountered poor weather while under VFR. The aircraft was approaching a mountain, but the pilot couldn't see it until it was too late. The pilot made a forced landing, and escaped alive along with the co-pilot.

The wreckage wasn't salvaged and the plane remains mostly intact.
