H&S Reimbursement Form

• If you are requesting reimbursement for multiple committees or events, please fill out a separate form for each one.

• Submit to the H&S Treasurer via the Main Office 

• If there are more than 3 receipts, please use a sheet that shows each receipt amount and the total

H&S Deposit Form 

• Please contact the H&S Treasurer when monies are ready to be deposited 

• Deposits can be submitted to the Main Office to be kept safe 

• Be sure to include this completed form with all deposits  

• If making a deposit from multiple committees or events, please use a separate form for each

Mini-Grant Application

• Grants are available to staff members to fund new and innovative ideas that will enhance Arrowhead’s current curriculum

• Please attach any supporting documentation (ie:  pictures, catalog page copy) you feel would be helpful with your application

• Applications, once signed by Dr. Roberts, can be submitted via our mail drawer in the Main Office

• Applications will be considered as they come in, rather than at a one-time review 

• Applicants will be notified by e-mail after Executive Board review

PBIS Grant Application

• Grants are available to PBIS Core Team members to fund ideas and incentives that will support the PBIS (Positive Behavior Interventions & Supports) program at Arrowhead

• Please attach any supporting documentation (ie:  pictures, catalog page copy) you feel would be helpful with your application

• Applications, once signed by Dr. Roberts, can be submitted via our mail drawer in the Main Office

• Applications will be considered as they come in, rather than at a one-time review 

• Applicants will be notified by e-mail after Executive Board review
