School Supply Order FormPlease use this form to order school supplies for each of your students. Please submit your credit card payment HERE or send a check to school office made out to ANDALE HSO. ** Please note earbuds/headphones, wireless mouses, calculators and trappers/binders are not included in this supply order. These will need to be purchased separately. ** **New this year: paper & cardstock are required supply items; they are INCLUDED in your kit.** ** Additional $5 donation for supply replenishment is not included in cost of Preschool kits. ** **If you have issues with the payment link on your phone, try using a computer.**
