Alodoc Inc ("Alodoc", "we", “us”, “our”) respect your privacy and are committed to protecting it by complying with this Privacy Policy (“Policy”). When you register with Alodoc as a user (“User”, "you", “your”), you become eligible to use the Alodoc Services (as defined below) and Healthcare Services (as defined below).
Please read this Policy carefully, as it describes:
· The types of information we may collect or that app users may provide when you download, install, register with, access, or use the Alodoc Services (as defined below).
· Our practices for collecting, using, maintaining, protecting, and disclosing information.
In this Privacy Policy, the following terms and expressions have the following meanings:
“Alodoc Services” means collectively the Alodoc information technology platform consisting of applications, websites, content, products, and services and a physical location where Healthcare Services may be provided.
“Applicable Law” means in relation to Alodoc, Authorized Providers, or Users, the laws and regulations, including privacy laws and regulations to which each is subject.
“Authorized Providers” means collectively physicians registered to practice medicine in the province of Ontario (“Authorized Physicians”) and regulated health professionals other than physicians registered in the province of Ontario (“Authorized Non-Physician Providers”) who are registered to use the Alodoc Services to provide Healthcare Services within their scope of practice to Users.
“Healthcare Services[AM1] ” includes Users ability to a) store certain Personal Information and Personal Health Information, b) share their Personal Health Information with Authorized Providers, c) electronically receive and store prescriptions, sick notes, and other documents, d) access certain health, telehealth, and other health-related services in the province of Ontario, e) faxing a summary of the User’s consultations with Authorized Providers to a healthcare provider of the User’s choice, f) securely faxing prescriptions to the User’s pharmacy of choice, g) provide notice when the User’s prescription is ready for pick-up, h) opening an account for Users with a prescription delivery service, and i) where available, securely transmitting User information to a prescription delivery service.
“Information” refers collectively to the Users Personal Information and PHI.
“Personal Information” means as any information about an identifiable individual, which includes information that can be used on its own or with other information to identify, contact, or locate a single person as further defined in Applicable Law. Personal information does not include business contact information, including your name, title, or business contact information.
“Personal Health Information” or “PHI” means information about an identifiable User’s health or healthcare as further defined in Applicable Law.
“TERMS” refers to the Terms of Use (User Agreement) agreement between the User and Alodoc.
We will only use Information in accordance with this Policy unless otherwise required by Applicable Law. We take steps to ensure that the Information we collect about you is adequate, relevant, not excessive, and used for limited purposes.
This policy applies only to Information we collect or are sent through or in connection with the Alodoc Services. For Clarity, this Policy DOES NOT apply to the collection, use, retention, disposal, destruction, or protection of Information by Authorized Providers through the Alodoc Services. This Policy also DOES NOT apply to Information that:
· We collect offline or on any other company apps or websites, including websites you may access through the Alodoc Services.
· You provide to or is collected by any third party.

Please read this policy carefully to understand our policies and practices for collecting, processing, and storing your information. If you do not agree with our policies and practices, do not download, register with, or use the Alodoc Services. By downloading, registering with, or using the Alodoc Services, you indicate that you understand, accept, and consent to the practices described in this Policy.
This Policy may change from time to time and your continued use of the Alodoc Services after we make changes indicates that you accept and consent to those changes, so please check the Policy periodically for updates. As required by Applicable Law, we will notify you in advance of any material changes to this Policy and obtain your consent to any new ways that we collect, use, and disclose your Information. The most current version of the Policy is posted at xxxx.
If you are a User, the arrangement between you and Alodoc, your rights and obligations and those of Alodoc, are set out in the Terms and, where applicable, the Supplementary Terms (as defined in the Terms).
If you are an Authorized Provider, your arrangement with Aloldoc, your rights and obligations and those of Alodoc, are set out in the agreement between you and Alodoc or, if you are providing Healthcare Services on behalf of a third party organization that has contracted with Alodoc, in the agreement between you and the third party organization with which you are associated.
We collect Information about you through:
· Direct interactions with you when you provide it to us, for example, by filling in forms or corresponding with us.
· Approved third party disclosures, for example, by requesting lab results be shared with us.
· Automated technologies or interactions, when you use the Alodoc Services, for example, usage details, IP addresses, and information collected through cookies, web beacons, and other tracking technologies.

The types of information that we collect includes:
· Personal Information that we can reasonably use to directly or indirectly identify you, such as your name, mailing address, email address, telephone number, internet protocol (IP) address, username or other similar identifier, billing and account information, social insurance number, and any other identifier we may use to contact you.
o We provide an opportunity for any user to unsubscribe from the Alodoc Services or opt-out of contact for marketing purposes an ongoing basis by accessing xxx, using the unsubscribe mechanism at the bottom of our emails, or emailing XXX.
· Personal Health Information that you provide before or during your use of the Alodoc Services and/or related Healthcare Services.
· Business-related information, namely information that is related to your business duties, including:
o title;
o duties;
o business address;

o business telephone number.]
· Non-personal information that does not directly or indirectly reveal your identity or directly relate to an identified individual, such as demographic information, or statistical, or aggregated information. We may derive non-personal statistical or aggregated data from Information. For example, we may aggregate information to calculate the percentage of users accessing a specific service.
· Technical information, including your login information, device type, time zone setting, and usage details.

Alodoc may disclose Information to the following sources or in the following circumstances.
Emergency Contacts. Authorized Providers may, but are not required to, contact your emergency contact(s) if they believe you are presenting with a healthcare emergency. It is your responsibility to advise emergency contact(s) that you are providing their name and contact information to Alodoc and Authorized Providers and for obtaining consent from said emergency contact(s) for the provision of said contact information in connection with the Alodoc Services.
Legal Requirements/Compliance/Fraud Prevention. Information may be disclosed to the extent required to comply with Applicable Law, to establish or exercise Alodoc’s legal rights, and to prevent or take action regarding illegal or prohibited activities.
Business Transactions. Alodoc will comply with all Applicable Laws may disclose Information for the purpose of required due diligence or for the completion of a business transaction, such as a merger, acquisition or asset sale.
Third Parties. With consent of the User, Alodoc may disclose Information to third parties. For example, we may disclose Information to the extent needed to an insurer or similar third party for the purpose of facilitating payments.

To protect your privacy and Information, we may request information to verify your identity when you contact us.
Your Information can be accessed, updated, or corrected at any time by logging into your account, except for any Information that an Authorized Provider has viewed or created.
To update or correct Information that an Authorized Provider has viewed or created requests should be made directly to the Authorized Provider.
We permit Alodoc personnel (“Personnel” which includes our employees, contractors and subcontractors) to access Information only to the extent necessary to perform their designated functions. We require Personnel to complete privacy and security training and to commit to protecting Information by complying with our policies, procedures, and Applicable Law.
We store your Information in electronic format within Canada, using computer systems with restricted access and that are housed in facilities using physical security measures.
More generally, we have in place appropriate physical, technological, and organizational safeguards, including access controls, to protect Information against loss, theft, and unauthorized access, use and disclosure.
Notwithstanding the safeguards we employ and our commitment to protecting Information, we cannot guarantee the security or error-free transmission or storage of Information. There are risks inherent in the use of electronic means to transmit and hold information in electronic format. These risks can be minimized but not eliminated by the use of appropriate security measures, such as the measures Alodoc employs.
We ask you to do the following to assist in protecting and maintaining the privacy of your Information:
Create a strong and unique password for your Alodoc account.
Do not share your account information or password with anyone.
We will never ask you for your password, including in any unsolicited communication such as letters, phone calls or email messages.
Log out of your account as soon as you finish using it.
Password-protect your device with a strong and unique password.
Choose a quiet, private location from which to receive any Healthcare Services.
Unless we notify you otherwise, Information will be retained until you (or Alodoc) close your account. On account closure, for a limited time, you will be able to print or copy your Information held by Alodoc, except for any private notes of Authorized Providers. Any request for access to an Authorized Provider’s notes should be made directly to the Authorized Provider. If you are having trouble finding the provider you consulted with, our support team may be able to help at xxx.
After the time period during which you may print or copy your Information has ended, we will securely delete the Information associated with your account, except for any Information that Alodoc is required to retain under Applicable Law or Information required by an Authorized Provider in connection with the Healthcare Services provided.
Alodoc will delete the latter once the Authorized Provider makes a copy of the Information or terminates their account with Alodoc. When Information is deleted, it is removed from our active database. Any Information that remains in our backup is securely stored and isolated from any further processing until deletion is possible.
Maple obtains the authorization of Authorized Providers to collect and use non-identifiable data about their use of the Alodoc Services (“Data”). This may include, for example, non-identifiable information about the number of consults carried out by Authorized Providers on the Alodoc Services or the symptoms most commonly treated by Authorized Providers using the Alodoc Services.
Alodoc may use Data to monitor the compliance of Users and Authorized Providers with the Terms, to make the use of the Alodoc Services more accessible, enhance the experience for Users and Authorized Providers, and to design optional surveys for Users. We may also use or disclose Data for internal purposes or for duly approved research projects.
We will not use Data to attempt to (re)identify Users or for any other purpose prohibited by Applicable Law.
Alodoc Services use “cookies” and other technologies such as pixel tags and web beacons. We use these technologies to better understand the use of the Alodoc Services, analyze trends, and administer, personalize and improve the user experience for Users and Authorized Providers. For more information about our use of cookies and your ability to accept or decline our use of cookies, please refer to our Cookie Policy, posted at xxx.
Alodoc Services may contain links to websites that we do not own or operate. The provision of these links is not an endorsement of or referral to the linked websites. We provide the links solely for your convenience. We strongly encourage you to review the privacy policies and terms of use applicable to any website you visit. This Policy does not apply to linked sites or pages and we are not responsible for the content or privacy practices applicable to them or used by their operators.
