HARTLAND COMMUNITY NURSES
The Community Nurses are available for home visits for older Hartland residents for assessment, support and referral to appropriate resources so residents can age in place, at home or at a residence of their choice.
The Nurse can be a contact for families who live far from Hartland, assuring them that all is well if it is and alerting them to issues or conditions that need attention. The home visits will be of continuing support to elders returning to their Hartland homes after a hospitalization or rehab. The Community Nurses do not provide direct medical care.
One of a growing number of Community Nurses in the Upper Valley, our program is supported by the Town of Hartland, Upper Valley Community Nursing Program, the Ottauquechee Health Foundation and private donations. We welcome your referrals and financial support. Please go here to offer your support.
Be part of the team that does not let any Hartland neighbor “fall through the cracks!”

Community nurse: Nancy North
Nancy North started as a community nurse for Hartland in June 2021. She is a retired Dartmouth Hitchcock nurse who decided to explore parish nursing, a specific sort of community nursing. She is pleased to put her training into good use with Aging in Hartland. Nancy can be reached at firstname.lastname@example.org or by calling 802-674-4118.
Community Nurse: Katie Williams
Katie started her nursing career in 2008 after graduating from Simmons College. She worked in maternal-child health through the Dartmouth and VNH networks for eleven years. In 2019 she began working with Aging in Hartland. Katie lives in Quechee with her two children and extended family on an old working Vermont farm. She loves community based care because it allows for relationships to be the first focus of the nurse-patient connection. You can find Katie by email at email@example.com or by calling 802-674-4118
