Download the HMO Reimbursement Form.
Complete all the needed information in the HMO Reimbursement Form.
Print and sign the HMO Reimbursement Form.
Submit the signd HMO Reimbursement Form along with the original receipts to Ms. Ma. Hazel Lequin (FMD) or Mr. Charlemagne Valdez (ADM).
You may follow-up after 1 week for the update and progress of the reimbursement request.
