The institution has a clearly identified and accountable governance structure, which delineates authority for the approval of institutional policies and responsibility for the overall direction and effectiveness of the institution. This management structure ensures the integrity and capability of the institution and its compliance with statutory, regulatory, and accreditation requirements.
Question A-1: What is the legal nature of the entity under which the institution operates? Provide copies of relevant institutional documents, such as certificates, charters, articles of incorporation, or partnership agreements. Include documentation pertaining to any dba (doing business as) name. Describe the ownership structure and list all owners of the institution, indicating the percentages of ownership held by each. If the institution is governed by a board of directors, provide a list of current board members. In the case of a publicly traded corporation, include a copy of the latest 10K report provided pursuant to SEC regulations.
Answer: Hail College of Technology (HCT) is a governmental organization that belongs to TVTC. In addition, HCT is a government corporation that has an administrative and financial dependence. See attachments 2-A-1-1 and 2-A-1-2.
Question A-2: Describe any external authorities that provide oversight of or guidance to the operation of the institution (e.g. state boards, licensing agencies). Provide copies of relevant documents including either a current state license or an exemption letter with regulatory reference, as applicable.
Answer: HCT is governed and guided by TVTC, which is also directed by the TVTC's Board. See attachments 2-A-1.1 & 2-A-2.
Question A-3: Identify any other name under which the institution has operated, been accredited, or been licensed within the past ten years, and indicate applicable time frames.
Answer: HCT's name is still the same since its foundation.
Question A-4: List any other organizations that currently accredit the institution and identify the type of accreditation granted (e.g. programmatic, institutional, etc.). Attach a copy of the current accreditation grant letter/certificate.
Answer: HCT is accredited by ACCET until 15th of December, 2018. See attachment 2-A-4.
Question A-5: Describe the extent of the services provided to the institution by outside legal counsel, a CPA, and/or other consultants, as applicable.
Answer: HCT is revised annually by TVTC by Certificate Accountant Auditor Office. This office audits HCT's financial records to insure that they are complying with TVTC standards.
Question A-6: Identify the positions that comprise the senior management team located at the main campus of the institution or the corporate office. Describe the individual responsibilities, interaction, and reporting relationships within senior management. What is the relationship of senior management to the ownership/board of directors.
Answer: The senior management team is the executive of HCT where it manages the college in accordance to TVTC's statute. See attachments 2-A-1-1, 2-A-6-1(Arabic), 2-A-6-2, 2-A-6-3(Arabic), 2-A-6-4, 2-A-6-5(Arabic).
Question A-7: Describe the process by which institutional policies are developed, reviewed, and revised, including who is responsible for developing policies and who has authority to approve them. If applicable, identify the role played by corporate boards, advisory groups, and/or outside consultants in policy development.
Answer: The general and main policies are developed and issued by TVTC and then sent to training units. However, HCT, like any other TVTC college, can develop its own policies through a series of procedures starting from the college to TVTC regional office until finally approved by TVTC. For example, if department-heads would like to implement a new policy, they would need to set up a meeting with the Vice-dean for Training. He will then report the suggestion to the Dean who would contact the General Manager to present the proposed policy to the Vice-governor for Training. Finally, the proposed policy will be presented to the Governor for approval and later on distribution to TVTC training units.
Question A-8: Who ensures compliance with appropriate regulatory, statutory, contractual, and accreditation requirements, and by what process?
Answer: Previously, the quality assurance administration was in charge, in the future there is a tendency to transfer the responsibility to the National Centre for Technical and Vocational Accreditation.
Question A-9: If applicable, who is the Principal Designated School Official (PDSO) for the organization? How many additional Designated School Officials (DSOs) are on staff?
Answer: Not Applicable.
Question A-10: If applicable, how does the institution stay current on immigration issues? Who is responsible for monitoring operations to ensure compliance with DHS regulations?
