Provide the CSC RO VI access to agency data, job-aids, information and other secondary data deemed necessary for the training program;
Ensure the punctuality and 100% attendance of not more than forty (40) participants in the said program;
Submit to CSC RO VI the final list and profile of participants at least five (5) working days before the start of the program;
Provide logistical requirements such as venue conducive for learning, workshop activities and reflection; audio-visual equipment, meals and snacks, training materials and handouts;
Provide transportation and accommodation for the resource persons/facilitators/training administrators, from travel time the day before up to travel time the day after the program;
Assign a personnel who will act as the focal person during the pre- and post-training stages, monitor attendance of participants, document the program and assist the CSC RO VI for its duration;
Submit to CSC RO VI the following documents in a long-size folder with tabs within five (5) working days after the end of the program:
Daily attendance sheet signed by participants;
Computerized list of participants with the corresponding actual number of training hours attended;
Post-training evaluation forms filled out by participants; and
Record of submission of Learning Action Plans by the participants.
Pay the CSC RO VI the training fee of P20,000.00 per day within five (5) working days from the conduct of the training. The training fee covers the cost of program management and honoraria of trainers. Check should be payable to Civil Service Commission and deposited to Training Fund LBP Account No. 0032-1025-49.
Pay the amount equivalent to 2% of the training fee for each participant in excess of forty (40). However, in no case shall the total number of participants exceed fifty (50). All payments shall be subject to COA rules and regulations;
Submit to the CSC RO VI Human Resource Division a copy of the Official Receipt upon payment of the training fee;
Require participants to submit their Learning Action Plan (LAP) to the agency’s HR department/division within three (3) working days from end of the training; and
Submit to the CSC RO VI a report on the implementation of the LAPs within three (3) months from end of the training.
