Note to UTK Students: Thanks for visiting the ProCADS site! This particular page of the site exists for a conference presentation in March 2018. Fill out any forms you want - these are samples to show others how our ProCADS system works. However - if you are looking for an "official" ProCADS form to fill out - they aren't here! Contact your professor or supervisor:)

AACTE Attendees: Greetings! This page has several links that you may look through in reference to our presentation on ProCADS at AACTE 2018. These ProCADS forms are examples that you may just glance through, or you can fill them out and see how the data is collected and organized. Remember - your responses will be visible to all others looking at these examples, too. We will keep this page active for a few weeks following AACTE, but will then remove it and delete any "data" collected via these example forms. The rest of this site will remain active and available for you to reference.

The last link is a ProCADS Presentation feedback request. You may use this form to provide feedback to us directly. These responses will only be visible to us. On this form, you can also provide an email address if you would like us to share our presentation with you.

• This is a copy of the form that teacher candidates, mentor teachers, and university-based supervisors use to evaluate (or self-evaluate) our interns at multiple points during their clinical experience. Remember - if you fill this out, your responses will be visible to others for the purposes of this example. Feel free to use pseudonyms and/or leave certain fields blank.

• This is an example of how the data looks following completion of the above form. This view is not provided to students or mentor teachers. Program area faculty are able to view the results from their students via shared access on Google Drive.

• This is a sample of the "feedback forms" that automatically generate when an individual completed a ProCAD survey. The individual completing the form can enter an email address to route the form automatically as well. Faculty can use these forms when conferencing with students.

• When "unsatisfactory" is selected in the ProCADS Survey, completers are prompted to also complete a deficiency report. This report details the deficiency and provides guidance and expectations to remediate or rectify.

• Just like with the "feedback form" this automatically generates when a deficiency report form is filled out. We've programmed the system to automatically the PDF to the person who submits the deficiency, and that person can enter additional email addresses to receive the PDF as well. We print a copy for the student and give it to them in the meeting to address the deficiency, and also keep a signed copy on file.
