GUIDELINES

Chapter 20. Student Clearance/Honorable Dismissal, Student Handbook.  Art. 1. Student Clearance. At the end of each semester, all students are required to secure clearance. Transfer credentials and other official documents shall be issued only when the students are cleared of all money and property accountabilities. 

For enrolment purposes, all undergraduate students need to have their clearance signed by the Office of Student Affairs. The students of USTP-Balubal Campus are hereby requested to adhere and act in accordance with the condition(s) and demands of the office to the students upon signing with the clearance.

The clearance is a certification that the student is cleared of all money and property responsibilities in this University.

 REQUIREMENTS

The following are the required checklist for the students to be fully cleared in the Office of  the Student Affairs and Services:

1. University Standing

   • 1. • The student must not have any pending sanction in the campus.

2. Tablet Evaluation

   • 1. • This is for those students who have availed the “Support for the Flexible Learning Program”.

        • The student must present the Tablet to the designated SAS officer, to evaluate the status of the gadget. Moreover, the gadget will be returned right after the evaluation.

          • 1. 1. The evaluation consists of its usability – if it is utilized for the Online Learning – of the tablet, as well, as the verification of its IMEI No.

               2. The IMEI number can be found in the following: Settings > System > About Phone > IMEI number

3. NSTP Serial Number

   • 1. •  Fill-out the form to be given by the SAS officers. However, the online form will be utilized ONLY by the freshmen.
