Transfer Credential for Graduates
Requesting of Transcript of Records (Form 137 - Grade 7 to Grade 12) for UST SHS graduates only.
INSTRUCTION: For UST Senior High School graduates requesting for transfer credentials (Transcript of Records / Form 137 - Grade 7 to 12), if the documents are intended for college admission or to comply with college deficiency, kindly secure an official request letter from the said university for us to process and address the documents to them.
NOTE: The Google form is open during office hours, Monday to Friday, from 8:00 AM to 5:00 PM ONLY.
Request for Honorable Dismissal
This form is used for students who will transfer to another school/institution. Accomplishing the Honorable Dismissal form is a prerequisite for the releasing of transfer credentials such as Form 138 and Form 137.
INSTRUCTION: Kindly submit the accomplished form to office.shs@ust.edu.ph together with a letter signed by your parent/guardian and their valid ID.

Dropping of Courses
This form is for students who are enrolled but is deciding to drop their subjects for the whole term. This form may also be used if a student is planning to file for a Leave of Absence (LOA) and wishes to re-enroll for the following term.

Refund Form for DepEd Voucher
INSTRUCTION: Please check if the voucher amount is already reflected on your myUSTe Portal under Billing/Fees tab. You may opt to deduct the voucher on your next enrollment in UST or you may request for a refund. To request for a refund, kindly submit the accomplished form to our Accounting Representative, Mr. Rhanny Lansang, at rhanny.lansang@ust.edu.ph.
