The application for make-up examination only applies to CURRENTLY ENROLLED STUDENTS who have failed to take the exam/s on the scheduled date for a valid reason.

Step 1: Accomplish this Google Form.
Step 2: Wait for an email reply from the Office of the Faculty Secretary for further instructions, if needed.
