Evaluation Process   

Program Evaluation and Timetable

• Fellow will meet with Program Director on a monthly basis for advising and feedback. Teaching faculty will complete this form to evaluate the fellow.

• Fellow will also evaluate the individual rotations and teaching faculty by completing this form at the end of each rotation. 

• The Program Coordinator will facilitate the timely dissemination and collection of these forms and review meetings.
