The Guidance, Counseling and Career Services Office is creating an online database of your records. To ensure that your information is updated, we are requesting you to fill up the Google Form Cumulative Record by clicking the link below the pubmat.

We would also like to ask you to fill up the Needs Assessment Form in able for us to know what services you might need and how we can provide the assistance you might need.

Also, we would like to remind you of the following:

1. Make sure to read and provide the information in every section.

2. After you answer the Cumulative Record, kindly proceed to answer the Needs Assessment Form
