One copy of this form will be submitted on behalf of the self faciliated PLC for each inservice day that they meet.

 Thank you for participating in May 27th Inservice Day! We hope you can take a few minutes to share your feedback about your experience. Your input will be used to inform future building base days, TRSU workshops, trainings and experiences.  After submitting your feedback, you will receive a confirmation message that allows you to download your completion certificate.
