How to Enter Behavior Referrals in Synergy

For Teachers – Step-by-Step Guide
Use this for serious or repeated behaviors that require administrative follow-up.
Log in to Synergy.
Find the student’s profile.
Click the dropdown menu (usually a list of options next to their name).
Select “Add Major Incident.”
You’ll be on the “Add Major Incident” screen.
Incident Date – Use the calendar picker.
Incident Time – Use the clock icon to select.
Referred By – Auto-fills with your name.
Refer To – Select an admin or counselor to handle the referral.
Incident Context Code – Choose where/when it occurred (e.g., During School – Lunch).
Location – Be specific (e.g., classroom, cafeteria).
Violations – Select all that apply from the dropdown.
Description – Write a clear, factual account (what happened, who was involved, what action you took, if any).
Click Next through the tabs:
Students – Confirm the student(s) involved.
Results – Add any outcomes (optional).
Create – Submit the referral.
Use this for less severe behaviors that are typically handled in class.
Navigate to the student’s name.
Click the dropdown.
Select “Add Minor Incident.”

Description – Include what happened and any classroom consequences you used.
Minor incidents are usually tracked for documentation and patterns, not for immediate office referral.
Once the form is filled out:
Confirm the details.
Click Save/Submit.
Your entry will be saved and viewable by the admin.
