General Guide Introduction


This Google Document is a general guide. It is not considered to be the final document, and it is not a legal document.


I am not providing legal, medical, or care giving advise in this document. It is only a general guide that has worked for me with my family.


You must adjust all of the individual sections of this document to suit your specific needs.


You should seek counsel from medical experts and/or legal experts regarding how to use this general guide.


My family created this document to help my Mother-in-law's CareGiver team be aware of the important aspects of our business relationship, as well as the vital components of Mom's care and their shift.


In our document, we replaced the word Client with our Mom's name and Representative with my Wife's name, to make this document more relevant/personal to our caregivers.


I share this for others to consider how they will write their CareGiver agreements, and as an idea of how to let the caregivers know about their shift and the important tasks they are required to do.


We put the shift Schedule information in the on-site handbook that includes other important information, including the daily log book we use to record what Mom eats/drinks and other important information and notes.
