Medication At
School
All students requiring medication to be taken during school hours will need to provide the Medication Administration Form completed and signed by the doctor and signed by the parent.
Students needing to carry an inhaler at school will need to provide the Asthma Action Plan Form completed by the doctor.
Students with food or other allergies requiring the emergency use of an epi-pen will need to provide the Allergy Emergency Action Plan and Treatment Form completed by the doctor.
Students requiring a food substitution by the cafeteria due to medical reasons will need to provide the Physician Statement for Food Substitution Form completed by the doctor.
