Complete the ON-LINE Registration using the INFINITE CAMPUS (IC) portal. CLICK HERE.

*If you forgot your username/password you can reset your info. 

**MAKE SURE TO FILL OUT THE EMERGENCY AND HEALTH information thoroughly!

1.  COPY (PDF/Scanned form, NOT a picture image OR DROP OFF at the school) your COMPLETE immunizations to the Nurse (only incoming 9th graders or NEW to OLH). 

   2. Fill out the Nurse's Medication and Emergency form  (only incoming 9th graders or NEW to OLH).

**Students cannot be enrolled unless the immunizations are up-to-date. CALL YOUR PROVIDER/Doctor to make sure OR scan the QR code to have access to the CALIFORNIA VACCINTATION PORTAL.

Address Changed??

 Complete the Affidavit ONLY if you have changed your address. 

Then email the following 

1. Completed Affidavit 

2. Current bill (see affidavit for acceptable documents,* joint residency has additional requirements)

3. Registration Confirmation Number (provided when On-line registration is complete in the Infinite Campus Portal)
