Include the following:  *Student's LAST name, First name

  *Address

*Date of Birth

                                                            *Grade Level the student is entering for the 2024-2025 school year

    *Phone number(s) to reach you in case we have questions

    *Do you have another student in Sweetwater Union (SUHSD) that is part of your household/family?  Yes   OR   No

  *Does the student have an IEP or a 504 plan? Please provide a copy/attachment.
