*If you forgot your username/password you can reset your info. Fill out the following Google Form.

**MAKE SURE TO FILL OUT THE EMERGENCY AND HEALTH information thoroughly!

Address Changed??

 Complete the Affidavit ONLY if you have changed your address. 

Then email the following:

1. Completed Affidavit 

2. Current bill (see affidavit for acceptable documents,* joint residency has additional requirements)

3. Registration Confirmation Number (provided when On-line registration is complete in the Infinite Campus Portal)
