The SES office will contact you by email within a day or two of completion of this form to pass on further directions in the enrollment process.
New students will also have to provide a
COPY of their birth certificate,
proof of residence (copy of utility bill with parent/guardian’s name), and
immunization records.

Home Language Survey
Free/Reduced application.
Student JOM Form: This form needs to be completed for Native American students.
Student Transportation Form: This form needs to be completed for NEW bus riders.
-Some kids are behind on immunizations due to COVID, please work with your family medical. person to get the immunizations caught up by kindergarten. Please submit to office the list of what you do have for them already.
-If you have chosen to not have your child immunized, please fill out an exemption form with the school office (it only takes a couple of minutes).
