Part 1

Part 2

Below you will find all additional forms and documents required for each student wishing to enroll in the SPS. Click on each link below to download the forms. Note that there are both hard copies to be printed and filled out as well as more online forms to complete and submit (see all 10 items below). Thank you!

    1. Parent Registration Checklist (Please use this checklist!) - Registrations will be incomplete and put on hold if anything is missing.

2. Health Record Screening Form (Print and Bring in a copy for each student.)

3. Parent Information Form (Print and Bring in a copy for each student.)

4. Military Form (For Military Families ONLY. Print and bring in, if applicable.)

5. Online Form: Home Language Survey Form (Click, complete and submit for each student. No need to print.)

6. Online Form: New Student Transportation (Must be completed for each family by clicking on link. (No need to print.)

7. Preschool Inventory Form: (Print for Kindergarten Registrations ONLY.) This form is to be filled out by the CURRENT preschool teacher and mailed into Early Childhood Office at 40 Fairbank Road, Sudbury, MA 01776.

Important Forms to Gather:

8. Copy of your child’s Birth Certificate or Adoption Decree

9. Proof of Sudbury Residency (POR) - (See details below for the three required documents, and read carefully.)

10. Copy of Doctor’s Physical form (within the last 13 months) & Immunization Records (birth-to-date). These forms are computer generated from your child's pediatrician. Here is the link regarding these forms: Immunization Information Forms .

For Grades 1-8: Please call the applicable school(s) to schedule an appointment and bring in all the above required documents.

For Preschool or Kindergarten students: please come to the Early Childhood Office at 40 Fairbank Road and bring in all the above required documents (on Tuesday or Thursday between 9 am and 3 pm).

**PROOF OF SUDBURY RESIDENCY**

This MUST be completed in person before a student is allowed to begin school.

The following three documents are required as proof of being a Sudbury Resident. Please note that post office box addresses will not be accepted.

HOME OWNERS: Please bring in photocopies of the documents below.

    1. Evidence of Residency: record of most recent property tax bill, mortgage statement, deed, or signed and fully executed Purchase and Sale (P&S) Agreement along with a signed statement by a Bank Officer or licensed Real Estate Broker certifying that financial commitment as been approved.
    2. Evidence of Occupancy: a utility bill issued within the last 60 days (fuel, electric or water bill). In the case of families who have not yet moved into the property at the time of registration, a bill may be supplied within 30 days from the student’s entrance date. Should a bill not be provided, students may be required to withdraw from school.
    3. Evidence of Identification: a photo ID of parent/guardian (valid driver’s license, valid passport, or other government issued ID).

RENTERS: Please bring in photocopies of the documents below.

    1. Evidence of Residency: a Lease and/or Rental Agreement signed by both the landlord and tenant, indicating the names of all tenants, and the effective dates of the agreement OR If Lease or Rental Agreement is not available, then a legal, notarized affidavit from the landlord/property owner affirming tenancy and address where student resides majority of the time will be required.     2. Evidence of Occupancy: a utility bill issued within the last 60 days (fuel, electric or water bill). (When a lease expires, a new new lease must be shared at the front office of each school.)

If you do not have access to a printer, please come into the Central Office: Department of Student Services at 40 Fairbank Road to pick up a hard copy. Registrations MUST be 100% complete: Missing forms will hold up the process of registering your child.
