Registration for School Year 2020-21 is upon us! Everyone is required to fill out a health history form. PLEASE be as accurate as possible as it assists me in caring for your child and keeps them safe at school.
As for immunization records, see below for the information pertaining to your circumstance:
CURRENT STUDENTS: If I have NOT contacted you specifically regarding immunization updates, then your child is up to date and I do not need a new record. I have their current one in their file.
NEW STUDENTS: Please see the attachment outlining DoDEA requirements. I will need a copy of their immunization record.
5th GRADERS GOING INTO 6TH GRADE: All 11 year old students are required by DoDEA to have a tetanus booster and a dose of meningococcal vaccine. If starting 6th grade at Spangdahlem or another DoDEA school next year, they will not be able to start classes if they do not have current immunizations. If your student has not received the required vaccines or hasn’t yet turned 11, I notified you via e-mail outlining the steps to take. Please feel free to contact me (cynthia.less@dodea.edu) if you need further guidance.
As a reminder, part of my duties as school nurse is to administer medications to students who require them. I will need a new medication order form at the start of next year to continue medications prescribed this school year. In order to give medication at school, DoDEA requires the following:
Medication must be accompanied by Form H-3-2, Department of Defense Education Activity Medication During the School Day/School-Sponsored Activities completed as follows and returned to the school.
a. Prescription information completed and signed by the physician/provider.
b. Consent signed by the sponsor/parent/guardian.
Prescription medication must be brought to school in the original container, labeled by the pharmacy, stating the name of the student, the medication, reason for administration, dosage, route, time of administration and the date issued. Prescription medications purchased as an over-the-counter medication and not subject to a pharmacy label, must be brought to school in the original unopened container labeled by the sponsor/parent/guardian with the student’s name, date of purchase and reason for administration and accompanied by the signed prescription.
