Google Permission Slips

Most families have previously completed a permission slip, but Google has changed some terms of service on our Google Accounts and Tools that now require Stockton Unified to obtain newly written permission from parents and guardians.  These accounts are used daily by students to log into their Chromebooks and access their work in Google Classroom and in our curricular programs. 

The new permission slip will be completed online.  Parents and guardians that have an email address in Synergy will receive an email to their personal email addresses.  The form will be pre-populated with the student’s name and information requiring only a review of the document and a signature from the student and parent or guardian. 

For those families without an email address in Synergy, blank forms will be distributed.  Parents and guardians will need to complete the contact/demographic information for their students, review the document, have both student and parent or guardian sign the form, and submit it electronically.

La mayoría de las familias del Distrito ya han firmado un permiso, pero Google ha cambiado algunas reglas de servicio en nuestras cuentas y herramientas de Google —y ahora requieren que Stockton Unified obtenga un nuevo permiso por escrito de los padres o tutores legales. A diario los estudiantes usan estas cuentas para iniciar su sesión en sus Chromebooks y abrir la página con su trabajo en ‘Google Classroom’ y la de nuestros programas curriculares.

Este nuevo permiso se completará en-línea. Los padres y tutores legales que tengan su correo electrónico en Synergy recibirán un correo electrónico en su buzón personal. Este formulario se habrá llenado previamente con el nombre del estudiante y la información requerida, por lo que solo debe revisar que todo esté correcto y ser firmado tanto por el estudiante como por el padre o tutor legal.

Para aquellas familias que no cuenten con un correo electrónico en Synergy, se les distribuirán formularios en blanco. Los padres y tutores legales deberán llenar la información de contacto y demográfica de su(s) estudiante(s), revisar el documento, hacer que tanto el estudiante como el padre o tutor legal firmen ese formulario y que lo envíen electrónicamente.

Paper forms:

You do not have access to the assessment to enter the permission slip in Illuminate as we did previously.  With the changes to Google permission, we cannot accept paper permission slips.  We need the electronic file as that will be our documentation going forward.  We moved to the electronic form to absorb the impact on your site staff and on my team.  We cant have 40K sheets of paper floating around out there and need to move the permission slips into the digital format.  I included some ways in the emails below to assist with ways to collect the digital forms from families that are concerned about completing online.  We have had a couple of sites that went quickly to printed permission slips and we had to return them all for the reasons above and that they were missing information.  PLEASE, Please, please talk with us before venturing off the path described above.
