St. Albert Youth Ministry Permission Slips

Below you will find the permission slips for all current events for both our middle and high school youth groups here at St. Albert the Great. Please contact Emileigh Ledgerwood, Director of Youth Ministry & Confirmation at eledgerwood@stalbert.org or call the church office at 502-425-3940 ext. 116.

All forms are now on JotForm to help eliminate paper waste and make it easier for you to submit permission forms for events! However, you can still download and print a form and send a hard copy to me by either:

1. Dropping it off at the parish office.

2. For St. Albert the Great school kids, sending it through kid-mail.
