Athletics & Activities

Welcome to the Sts. Peter & Paul Athletics webpage. The Athletic program at Sts. Peter & Paul is an extension of the learning experience for our students.  We believe that participation in athletics provides students the opportunity to continue to develop physically, intellectually, emotionally, and socially in a Catholic environment beyond the school day.

Before participating in any sporting activities at Sts. Peter & Paul each student is required to have all of the following forms filled out completely. Athletic Forms can be obtained by clicking on the hyperlinks below.

First, read the Athletic Handbook for the current school year, fill out the form to acknowledge that you have read the handbook and agree to its rules and regulations. If you have any questions or don’t see a form you need, contact us at beddy@sppslex.org.

Fill out Handbook Acknowledgement Form Athletics and Athletics Handbook on page 10. Please return this to Mrs. Eddy.

Grades 5-8-Fill out pre-participation form print and take it and the Physical Form to the doctor and then turn it in to us.

Grades K-4 Fill out alternate Physical and Participation Form and turn it in to us.

The Athletic Director will give you the Athletics/Activities Permission Form with dates and cost at the Parent Meeting. You can view this form below.

Please be sure to turn in completed physical forms on or before specified dates. All participating students and a parent/guardian must sign and turn in the Sportsmanship Pledge and Acknowledgement page from the Handbook. Please click on the square with the arrow to download the forms.
