Certificated Performance Evaluation Forms

Temporary and Probationary Employees are evaluated each year. Permanent Employees who “partially meets standard” in standards 1-5 or “does not meet standard in any standard 1-7 are evaluated each year.
Permanent Employees are evaluated every other year.
Permanent Employees who have been employed for at least 10 years in the District and have met all standards on the previous evaluation may be on a 4-year evaluation cycle provided the employee and evaluator agree and indicate on the evaluation by providing the next evaluation date.
