Prepare all documents listed below with the appropriate number of copies.
Scheduled via the online booking portal (unless otherwise noted by the consulate - a select few allow visa applications by mail).
Appointments must be confirmed 10-3 days before the appointment is set to occur. Applicants must log back into the prenotami system and navigate to "My Bookings" to confirm their appointment. Applicants should not rely on the system to send them a reminder email for this, they need to set their own reminder to log in and confirm their appointment. Otherwise, they will lose their appointment.

One of the photos should be glued to your visa application. If you don't have a glue stick, use a paperclip, but do not staple.
The real thing. Depending on the consulate, your visa and passport may be returned to you via FedEx as long as you provide a pre-paid self-addressed return envelope along with your application materials. Please confirm on the consulate's website that this option is allowed.

Itinerary must show your name, and flight information including date of entry and border of entry. A printed receipt from an online booking is perfect, as long as your arrival city is spelled out; airport codes are not acceptable. The date of arrival in Italy and entry point written on your visa application must match your flight itinerary. The return date on your itinerary will not affect your visa application.
Download the Italian Consulate Visa Checklist for your reference; it does not need to be submitted with your application. You'll make sure to compile documentation as indicated below.
Notes on the Checklist:
Item #4: You can disregard. HOWEVER, please be sure to list your city and state in item #5 of the visa application form.
Item #6: This is the letter that you will receive by logging back into the UniversItaly platform as a result of your online University of Florence registration.
Item #7: This will be created by Angelo Pisano.
Item #8: You need to supply the most recent month's bank account statement. This is to prove that you have access to cover your personal expenses while you're in Italy. The recommended amount is about $30 per day (either in regular deposits to a parent's checking account with additional supporting documentation or in a savings account under your name).
Item # 9: Print your financial aid award letter.
Item #10: This will be created by Angelo Pisano.
Item #11: Money Order. Please make sure to purchase the money order from CVS in the amount outlined below.
Item #12: Use this template of the Affidavit of Support only if you cannot provide a bank statement in your name for Item #8 above. Note that this form must be notarized. Notary services are available at SFS or the Registrar's Office in College Hall or at Forbes Library. Notaries public must witness the signature happen in real time in order to validate them and add their stamp.
The Italian Consulate charges a fee to process visas, which is payable by exact cash or money order only. The amount of the fee changes quarterly (1 January; 1 April; 1 July; 1 October) and is determined by the Italian Consulate, but is the equivalent of €50. You must use the visa fee appropriate for the quarter in which your appointment is scheduled (e.g. if you submit your application in April, but don't receive an appointment until July, you must wait to get a money order or withdraw exact cash until the new fee is published). Please visit either a US post office or a CVS/Walgreens location to secure a money order payable to "ITALIAN CONSULATE IN BOSTON" at 600 Atlantic Avenue, Boston, MA 02210 in this amount and include it with your other materials.
