COPD is widely recognized as a serious and debilitating chronic condition, along with being one of the leading reasons for hospital admissions in the older adult population. The aim of this study was to determine if telehealth support and health literacy levels impact all-cause 30 day readmission rates to the hospital for participants with COPD over a six month period. Each nursing student actively recruited patients with COPD from SSM SLU Hospital to participate in the study, which involved health literacy screenings and biweekly telehealth calls over a 30-day period after discharge. None of the patients that participated in the study (with calls maintained over the whole 30-day period) were readmitted, demonstrating a positive correlation between telehealth interventions and decreased readmission rates. While not an entirely novel research question, this study aimed to build upon existing literature while addressing a serious issue within the healthcare system. By promoting health literacy in this chronic disease population, the nursing students served as patient advocates to create better patient outcomes and reduce the burden of chronic disease on the healthcare system.

After graduation, she will be working on the Labor and Delivery Unit at Mercy South Hospital in St. Louis Missouri and pursuing her goal of becoming a women's health nurse practitioner. Marissa enjoys working out, cooking, and spending time outdoors.
Sarah is a nursing major and Spanish minor from Cincinnati, Ohio. After graduation, she will be working in the Medical ICU at the Cleveland Clinic with hopes that experience in critical care will help her decide what advanced nursing degree she would like to pursue. In her free time, Sarah enjoys cooking, reading, watching sports, and exploring the city!
Savan is a nursing major from Hanover Park, Illinois. After graduating, Savan hopes to pursue a career in pediatric or maternal-newborn nursing with the goal of becoming a pediatric nurse practitioner. In his free time, Savan enjoys spending time outdoors, traveling, and finding new restaurants in the St. Louis area.
Sara, a nursing major from Nashville, Tennessee, is eager to pursue a career in pediatric nursing. Minoring in Spanish, she plans to further her knowledge of the language through volunteer work in Costa Rica upon graduating. She aspires to eventually obtain a nurse practitioning degree in pediatric nursing. Furthermore, Sara enjoys spending time outdoors, practicing yoga, and reading a good book.
